Community Health Workers (CHWs), known as Agentes Polivalentes Elementares (APEs) in Mozambique are essential and critical to deliver clinical and outreach services to remote communities in low- and middle-income countries to reach Universal Health Coverage. Community health information systems (CHIS) and mobile applications have been increasingly deployed to support CHW services. Largely implemented by partners, these tools can create community data silos that rarely feed into the District Health Information System (DHIS2) of the Ministry of Health (MISAU) or other health management information systems
